Instructions for Completion

If the person being reported is in immediate danger, please call the campus security hotline at 0937-295995 or contact the police at 110.
The center will contact the submitter by phone or email to understand the situation. If the status of the reported individual cannot be confirmed, the report will not be accepted.
The center’s counselor will assess the situation to determine whether to proceed with providing care.
The center collects your personal information for the purpose of conducting counseling-related activities and ensures the responsible protection of your personal data.
